Psychiatric/Psychological Appointments

Please complete the Agreement to Accept Orders of Psychiatric/Psychological Appointment form. Once completed, please submit the form and curriculum vitae via email to: doctorspanel@sb-court.org. You may also submit the documents via U.S. Postal mail to the following address:

You will be notified within four to six weeks regarding the status of your application.

Once your application has been approved, you will be contacted by the Contracts and Procurement Department.

All billing-related questions must be submitted to the district where the case is being heard. All other questions may be submitted to: doctorspanel@sb-court.org.
